FOR THE TRUE HEALTH FOUNDATIONS JUMPSTART KIT

This agreement ("Agreement") is entered into by and between reGenesis Health ("Company," "we," "us," or "our") and the user ("you" or "customer") who purchases the True Health Foundations Jumpstart Kit ("Kit"). By purchasing and using the Kit, you agree to be bound by these Terms and Conditions.

 

1. Purpose and Medical Disclaimer

The True Health Foundations Jumpstart Kit is designed to provide you with general, foundational information and insights related to your genetic blueprint. The report and any accompanying materials, including the initial practitioner analysis overview and roadmap report, are for informational and educational purposes only.

  • No Medical Advice: The information provided in the Kit and on our website is not a substitute for professional medical advice, diagnosis, or treatment. You should always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have read in the Kit or on our website.

  • Not for Diagnosis: The Kit is not a diagnostic tool and should not be used to diagnose, treat, cure, or prevent any disease. It is not intended to address specific health issues or conditions. The purpose is to provide you with information to help you understand potential foundational influences on your health and to aid in self-healthing efforts under the guidance of your own healthcare team.

  • No Doctor-Patient Relationship: Your purchase of the Kit does not establish a doctor-patient, practitioner-client, or other professional relationship with the Company or its practitioners.

 

2. Distinction Between Client Relationships

The relationship established through the purchase of the True Health Foundations Jumpstart Kit is a customer relationship. This is distinct and different from a private client relationship with a reGenesis Health practitioner.

  • Kit Customer Relationship: As a customer of the Kit, you are a "DIY self-healthing" user. The Kit provides generalized tailored information and an overview of your data as a starting point. There is no in-depth analysis of your personal health history, nor is a specific plan of care created for you.

  • Private Client Relationship: A private client relationship is a more in-depth, personalized, and clinical engagement that includes a complete health history, advanced analysis, and a focused plan of care, and requires separate legal agreements and paperwork. This type of relationship is not established through the purchase of the Kit.

3. Data and Privacy

We are committed to protecting your privacy. For the purpose of providing and shipping the True Health Foundations Jumpstart Kit, we will collect a limited amount of personal information.

  • Information Collected: We will only require your name, address, and phone number for general billing and test kit shipping purposes.

  • No Health Data Stored: We will not require or store your health history, medical information, or the raw genetic data from your test. The information provided in your report is processed by a third-party laboratory and is not maintained by the Company for personalized follow-up unless you choose to engage in a separate, formal private client relationship.

 

4. Customer Responsibilities

You acknowledge and agree that you are solely responsible for how you interpret and use the information from the Kit. You agree to:

  • Consult with a qualified healthcare professional before making any changes to your diet, lifestyle, supplements, or medical treatment based on the information provided in the Kit.

  • Use the Kit and its contents for legal, personal, and non-commercial purposes.

  • Not use the Kit for diagnostic purposes.

 

5. Limitation of Liability

To the fullest extent permitted by law, reGenesis Health shall not be liable for any direct, indirect, incidental, special, consequential, or exemplary damages, including but not limited to damages for loss of profits, goodwill, use, data, or other intangible losses resulting from your use of the Kit or any information provided therein.
